I could conceivably drive over and collect a bootful of PETs, if I knew where to go. And if it weren't TOO far.

I've ordered from http://www.hopfenkurier.com/bierspezialitaeten/helle-biere.html?dir=asc&limit=all&order=name a few times in the past, including getting either Gambrinus 10 or Krusovice 10 in PET bottles. (Yes, not your criteria above, but this is all I know of.) Though, that Bernard swing-top sounds interesting -- yeast added, secondary conditioning in the bottle. And I'd go for a few other bottles I see that are new (to me) there.

Gaststätte Röming here in the Altstadt has Krusovice dark on keg, as well as a rotating pale industrial Czech beer on every month, if you get desperate after work, not at all far from the Bahnhof. But he operates a restaurant and really only wants to serve diners, sadly, as it's a great old tavern.
