There are smaller casks available than 5 gallons, aren't there? The 5L Partyfass should be cask-able, right? IME / IMO, it's the fact that the beer is still sort of "alive" in properly casked form that makes a big difference. The beer is just softer and more interesting than otherwise, and changes over the course of the 2-3 days it's on tap. It will normally peak during this time, and the hit-or-miss aspect of finding a peaking beer is part of the charm of British pub crawling. Also, all this about cask-conditioned beer in the US...in Oregon, it used to often (if not mostly) be normally-kegged beer dispensed through a hand-pump, and not really cask-conditioned. I suspect your NERAX people do it the right way, and hopefully east coast brewers do it more often too.
